Day Opportunities Service

 

What is the Day Opportunities Service?
The Day Opportunities Service is a new project available to people with disabilities in the Western Health and Social Care Trust area.  It supports people with disabilities to participate in and contribute to their local community as an alternative to traditional day centre services.


It is a flexible service based on choice. It aims to help people find things that they would like to do in their community.  Our Project Officer will work with you to make sure you can access the services you want to.


How does it work?

A Project Worker will work with each person for up to 16 weeks. She will help to identify and source the supports required, to assist the person to take advantage of opportunities available in their local community.

 


A plan will be agreed to show how the person can achieve their goal and what support will be necessary.  The plan will also focus on opportunities the person can continue on their own at the end of the project.

How do people access the service?

A referral must be made either from the person themselves, a family member or a healthcare professional.  To get further information on the service or to enquire about making a referral please contact the Project Officer.
